This  patient  met with an accident during festival. A blast injury to his right hand. Professionally he was a tailor. A successful surgery with a vigorous physiotherapy and now rehabilitation back in society. The patient is still undergoing physiotherapy at Pratiksha

SO I SAY...
"Bhagwan bhi khush or sharir bhi khush"
At Pratiksha we always emphasize on surya namaskar as a home program for all patients specially LBA. Yes in certain cases we do modify the exercise as per patients condition. 
In chat puja we worship God surya and surya namaskar is worship of 12 names of god surya via 12 different yoga postures. As per hindu dharma worship of god surya helps you save from disease of skin (leprosy) and other disorders.

If we look at the scientific aspect, performing this exercise early morning is helpful in building vitamin D and a good stretching for the entire spine. I even recommend this in all neurological cases. Acting as one of the forms of meditation it help as an antidepressant as well as good for vestibular problems like vertigo, miners disease and pulmonary disorders like asthma and COPD. 

For my non Hindu readers we can co relate this exercise with nature . Panch Mahabhuta Tatwa (five Natural Elements), namely, Akash (Sky or Space) we perform this exercise in open space, Vayu (Air)early morning in fresh air, Jal (Water), Agni (Fire) signifies surya(sun) and Prithvi (Earth) as this exercise we perform on ground. These five Elements combine together to create a balance in Nature, which ultimately helps in the creation and sustenance of life

The day is not far when India is said to be running on shoulders of DISABLED
And this case is among few of them.
We all health professional need to think in a holistic way, when it comes to treating a patient.
Any approach is only app laudable if the patient is successfully rehabilitated in the society.
There is need  specialized centers for vocational training of  disabled helping them with rehabilitation.

FOR PHYSIOTHERAPIST
Being a physiotherapist , before admitting any PT regime we need to asses the TMJ joint and concentrate on the probable cause of dysfunction.
a. Movements to target for range of motion- depression(opening)
                                                                     - occlusion(closing)
                                                                     - lateral deviation left and right
b. The muscle around TMJ- mainly supplied by facial nerve and mandibular nerve need to be targeted
c. Cervical spine - condition like sleep apnoea may also be associated with TMJ and cervical spine
d. Functional assessment like- swallowing and psychological assesment with pain should be considered

A holistic approach with proper assessment of all components can only help in rehabilitating a patient with TMJ dysfunction
FOR GENERAL :
Early exercise for- stiffness
                            - your joint range of motion
                            - muscle health
Physiotherapy has big role to maintain above 3 . In case you find any problem in your jaw consult your nearest physiotherapist

Wake up Physios- Corporate hospital investigation- a financial burden to patient or a remedy from disease




Above mentioned report is of an ultra sonography of shoulder. This person had a shoulder injury and was advised for CT,MRI. I did a clinical examination and sent him for ultrasound and here is the report.

Is corporate culture making doctors forget there ethics or we just have stopped reading books. An MRI can give more monetary return to a doctor.

Dear physios we are mucle kinesiology scientists. Rather movement scientist. Improving our radiodiagnostic methods can help us make patient recovery early. We can treat our patients without burden of heavy investigation and financil burden of costly drugs.
 

I rehabilitated the patient in 3 sittings With a home program for a week.

Good bye combiflams,paracetamol and foot insoles/orthosis- simple physiotherapy can help cure foot deformity

Many at times I came across cases in school children complaining of foot pain in orthopedic clinic. Prescription of pain killers or max a foot insole is the ultimate solution. My Question is in olden days when we walked bare foot,was there any insoles needed. Bare foot walking strengthened our foot muscles and non sedentary lifestyle kept our spine healthy.
With 5 sessions of physiotherapy displaying result in a 12 yr girl complaining of foot pain.

For my physio friends- Integument physiotherapy is a very up coming branch in physiotherapy in India. Many cases here I treat may be called a neurologically affected case but mainly it needs physiotherapy of skin. Many newly pass out physios can tke up this branch as a post graduation subject. Preventive Against Physiotherapy


Today met a patient for whom physiotherapy was not at all required . Taking history I came to now she had taken physiotherapy for around 1 month but no improvement was visible. I started taking history and it made me conclude that she was in need of medication rather than physiotherapy. 
This bring s me to think, how many of physio professionals be true to our profession and say no to physiotherapy for few cases we come across in our practise

I remember Dr Deepak kumar s mulligan classes. Before we were leaving by the end of course he said "we are not god". So true we cant treat everything. At times a direct no helps you build confidence in your patient. The putting the below image i want to say I am not just a physiotherapist rather I am a good physiotherapist

I will be uploading few cases of mine which did not require physiotherapy. So I call this blog as         "Preventive Against Physiotherapy"

Perthes disease in 20 yr old girl

X-Ray of a 20 yr old girl
























Reshmi (name not revealed as per request of patient) came with a complain of inability to abduct her hip beyond 30 degrees. Flexion  of hip possible till the range of 50 degrees.This girl was put on cast post trauma to the pelvic region. X-ray reveals reduced space between acetabulum 
and femur head. Avascular necrosis of articular surface.

Now my question - 
1. Age has not come to take her for any surgical management ( like THR)  so what  should a physio aim at?
 2. With time disuse atrophy can occur around hip muscles. so what should be the  PT management
 3. Psychological factors need to be considered. What is PT role in that & how?
 4. what shd be the short term and long term goal

Conclusion: 
Our management must be a sum up of all this . This helps you set up your own protocol, no need of any book

22 yr old with ankle pain

Boy preparing for army fitness test suddenly complains of pain in ankle. Pain s mainly in medial arch of both foot , more in right. On and off complain of pain in lower back also.
When  I examined him at pratiksha I could see foot in eversion with reduced arch.
Fine I prescribe him correct footware with customised insole but what about the pathology?
Will rest and anti inflammatory be a solution.
No the answer is treat the root cause
This requires coordination between physio, orthopaedician, prosthetics and the awareness of the root cause.
What u say is the root cause?
